NOTE: A VALID POLICE REPORT IS REQUIRED IN ORDER TO BE ADDED TO OUR DATA BASE.

In order to include a missing person on our site, we require the following information:

- A clear photo of the missing person

- The date of disappearance

- Age at the time of disappearance

- Date of birth

- Hair color

- Eye Color

- Height

- Weight

- Any clothing/jewelry the missing person was wearing

- Any identifying characteristics (tattoos, moles, etc.)

- Where pertinent, medical information that is necessary to help identify missing person (Alzheimer's Disease, etc.)

- Circumstances of the disappearance

- Name of the police department where the police report was made

- The contact telephone number for police department (to call with tips or information on missing)

- Your name:

- Your relationship to the missing person: i.e., mother, friend etc.

Once all information is completed and verified, we will place the missing person or persons on our web site.
