He has scars on his back and chest, a tattoo of a heart with wings and the name "Lindsey" on top of the wings on his upper left chest, a faded tattoo of the word "Harley" on his upper right arm, a tattoo of a cross on his left arm, and is missing his upper front tooth.

A black shirt, dark blue jeans, and black loafers.  Possibly wearing a gold wedding band.

Circumstances:

Jeremiah was living in Ludlow, Illinois at the time he disappeared and was possibly last seen on State Road 32 near Perrysville and State Road 63 in Covington, Indiana. His car, a blue and gray 1979 Toyota pickup truck, was located two days later on State Road 63 in Perrysville, Indiana.
